This page is used for ServiceMaster employees and/or agency assignees to submit their Certificate of Completion for ServiceMaster OHSA Health and Safety Awareness. Please complete the form below with all required information, and remember to attach the actual PDF file that contains your certificate, that you have received upon successful completion of the ServiceMaster training module.
